# How do I create a set of ratings for a question?

In order to view **Return to Work Declaration** forms and **Training Event Feedback** forms within **Self-Service**, you must first set up the forms within **Cintra iQ**. Each form should contain questions for the **Self-Service** user to answer. A **Rating Set** is used when you want to display a drop-down choice for a question response. These are:

1. Outstanding
2. Met expectations
3. Did not meet expectations, or yes/no etc

A **Rating Set** is the title of the header under which you apply individual rating records. What is entered in this pane will be selectable when you want to define the response to a question that requires a rating drop down for an answer. Within this pane, you can create a name for the rating set.

## To create a name for the rating set

### Navigate to the Ratings Tab

1. Go to **Cintra iQ**: System Administration> Self-Service Forms> Ratings tab.

### In the Rating Set Pane, Create the Rating Set

1. Click the **New** button. The **New Rating Set** form appears.
2. Enter the **Rating Set Name**.
3. Click the **OK** button. The record appears in the **Rating Set** pane.

### Highlight the Desired Rating Set

### In the Ratings Pane, Create the Rating to Attach to the Rate Set Name

1. Click the **New** button. The **New Rating** form appears.
2. Enter the name of the **Rating**. eg 1, 2, 3, or Yes, No etc.
3. Enter the **Sequence**, in which the rating appears in the list.
4. Enter the rating's **Description**. e.g. outstanding.

> **Note:** The drop-down list displayed to the user is a combination of the rating and the description. e.g. 1-Oustanding. If no additional description is required for a rating e.g. YES/NO, then leave blank.
5. Click the **OK** button. The rating appears in the **Ratings** pane.
6. Continue to enter the applicable ratings.
