Guide: Self-Service forms

In order to view Return to Work Declaration forms and Training Event Feedback forms within Self-Service, you must first set up the forms within Cintra iQ. Each form should contain questions for the Self-Service user to answer.

Steps to take

1. Ensure Self-Service has been set up to access the Return to Work Declarations and/or Training Event Feedback forms.

2. Create the question ratings in Cintra iQ.

3. Create the questions in Cintra iQ.

4. Configure the form in Cintra iQ.

5. Access the appropriate form in Self-Service.

The Training Event Feedback form is now obsolete and may not be available on your system.
